Questionnaire Design

          SDW's experts can work with you to develop a questionnaire that will not only meet your research objectives, but will also facilitate fast and accurate processing.

          A well-formatted document can make all the difference as to whether a potential respondent completes and returns your survey. Our trained staff makes sure that your survey will be presentable and easy to complete. If desired, questionnaires can be fully individualized. No job is too big or too small. We offer questionnaires in the following formats:

          The traditional survey where data is entered via keyboard entry is referred to as Non-Scannable. You can view an example here. This type of survey is typically ideal for low volume projects consisting of less than five thousand surveys.

          SDW also has the ability to capture data automatically by scanning thousands of single or double-sided sheets per hour. These surveys can range in size from as small as a postcard to custom-sized booklets. Multi-page forms are easily accommodated. Surveys can be completed with a pen or pencil.
